Waiver *

I have read the waiver listed below and I AGREE TO THE TERMS.

Because physical exercise can be strenuous and subject you to risk of serious injury or even death, we urge you to obtain a physical examination from a doctor before using any exercise equipment or participating in any exercise activity.

You agree that by participating in physical
exercise, or personal training, or group fitness training activities, you do so entirely at your own risk. By applying your physical or electronic printed signature below you are
inexhaustibly waiving your right to file any legal claim or action against JC1 Enterprises LLC, or it’s affiliates, or any instructors of JC1 Enterprises LLC (DBA: JJF Bootcamp and/or Joejitsu Fitness).
You also waive your right to file any legal claim or action against JC1 Enterprises LLC, it’s affiliates or any trainers/instructors of JC1 Enterprises LLC (DBA: JJF Bootcamp and/or Joejitsu Fitness) while participating in virtual fitness training. Online Training is at your own risk and it is strongly recommended you consult your physician before starting any type of online virtual fitness training.

Any recommendation for changes in diet, including the use of food supplements, weight reduction, and/or body building enhancement products are entirely your responsibility and you should
consult a physician prior to undergoing any dietary or food supplement changes.

You agree that you are voluntarily participating
in these activities and assume all risks of injury, illness, or death.

You also give permission for JC1 Enterprises LLC (DBA: JJF Bootcamp and/or Joejitsu Fitness) and/or it’s
affiliates to capture and use your image, whether photograph or video, in any manner seen fit for continued business purposes.

Non-Compete *

I have read the non-compete listed below and I AGREE TO THE TERMS.

You are also agreeing not to solicit any members of JC1 Enterprises LLC (DBA: JJF Bootcamp and/or Joejitsu Fitness) for personal, group, or online training and are agreeing not to
compete with JC1 Enterprises LLC (DBA: JJF Bootcamp and/or Joejitsu Fitness) within 20 miles of 201 Howell Ave. Brookville, Fl. 34601 with the formation of any adult group
fitness exercise class where you will serve as either a trainer, owner, or consultant within 6 months after your last day as a client, member,
or employee of JC1 Enterprises LLC (DBA: JJF Bootcamp and/or Joejitsu Fitness)

Any attempts to do so will negate the use of your time with JC1 Enterprises LLC (DBA: JJF Bootcamp and/or Joejitsu Fitness) as physical training
and instead identify your time with JC1 Enterprises LLC (DBA: JJF Bootcamp and/or Joejitsu Fitness) as business coaching for which you are agreeing to pay $1000.00 to continue doing said business within 20 miles of 201 Howell Ave. Brookville, Fl. 34601 if still within 6 months of your last day.

Any and all workout programs or any other documents you
receive throughout training services whether written, electronic or otherwise are property of JC1 Enterprises LLC (DBA: JJF Bootcamp and/or Joejitsu Fitness) and are not to be
shared outside of your client/trainer arrangement.

Any attempts to do so will negate the use of your time with JC1 Enterprises LLC (DBA: JJF Bootcamp and/or Joejitsu Fitness) as
physical training and instead identify your time with JC1 Enterprises LLC (DBA: JJF Bootcamp and/or Joejitsu Fitness) as business coaching for which you are agreeing to pay
$1000.00 for use of said properties.

Release of Liability *

I have read the release of liability listed below and I AGREE TO THE TERMS.

You acknowledge that you have carefully read this “waiver and release” and fully understand that it is a release of liability.

You expressly agree to release and discharge the trainers/instructors and other members of JC1 Enterprises LLC (DBA: JJF Bootcamp and/or Joejitsu Fitness) and its associated facilities from ANY and ALL legal
claims or courses of legal action. You are agreeing to voluntarily give up or waive any right that you may otherwise have to bring a legal
action against JC1 Enterprises LLC (DBA: JJF Bootcamp and/or Joejitsu Fitness) it's trainers/instructors, or the City of Brooksville Florida. This release of liability includes personal injury and property damage claims/cases.
To the extent that
statute or case law does not prohibit releases for negligence, this release of liability is also for negligence.

If any portion of this release of
liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall
remain in full force and effect and the offending provision or provisions severed here from.

By signing this release of liability, you acknowledge and understand its content and that this release of liability cannot be modified orally.

Finally you understand that you should request a copy of these terms via personal exchange or email within 72 hours of signing. If,
after reviewing, you find the terms unfair or different from what was explained to you, you will have an additional 72 hours to have
another form signed with appropriate terms or decide not to continue with the services.
